Advantmed is seeking a licensed Nurse Practitioner to conduct structured in‑home wellness and risk‑adjustment assessments as a 1099 PRN contractor. Typical visits are completed with autonomy and professional support, with about 4–5 visits a day based on your schedule.

Qualifications

• NP license active in applicable state.
• 3+ years of patient care experience.
• Proficient with EMR systems and able to work independently.

Responsibilities

• Conduct in‑home Annual Wellness Visits and chronic condition assessments.
• Perform histories, exams, vitals, medication reviews, and screenings.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value‑based care and risk adjustment.
