Job summary

Advantmed, LLC is seeking a licensed Nurse Practitioner for in-home wellness and risk adjustment assessments on a PRN basis (1099). The role offers flexible weekdays, no evenings or weekends, and paid mileage.

Expect ~4–5 visits per day depending on travel and location, with comprehensive training and EMR support. Responsibilities include conducting annual wellness visits, focused histories, vitals, and disease management education, documenting visits in the EMR to support value-based care and

Qualifications

• Active, unencumbered NP license in the applicable state.
• 3+ years of patient care experience preferred.
• Comfortable with EMR systems and independent work.
• In home or risk adjustment experience preferred.

Responsibilities

• Conduct in home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, medication reviews, and screenings.
• Screenings: Hemoglobin A1C, Spirometry, KED, DRE, FIT.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
