Job summary

Cooper Norcross Health is seeking a Mobile Integrated Health Care Provider to enhance in-home care and care coordination. The MIH model expands EMS from emergency response to proactive, patient-centered care, bridging gaps between EMS, primary care, hospitals and social services for at-risk populations.

The role includes in-home assessments, care coordination, chronic disease management, post-discharge visits, and education for first responders on SUD resources.

Qualifications

• 3-5 years of experience in advanced life support (ALS) environment as a paramedic.
• High School/GED is required; bachelor's degree preferred in healthcare or public health.
• Paramedic license/certification requirements with regional reciprocity awareness.

Responsibilities

• Conduct comprehensive in-home assessments identifying medical, behavioral, and social needs in the patient’s home.
• Coordinate with primary care, care managers, and specialty teams to close care gaps.
• Identify and connect patients to community-based health services, including primary care, public health, and mental health resources.
• Monitor patient status and escalate to 911 when needed; coordinate with Transitional Care and providers.
• Provide clinical care within NJ paramedic scope per NJAC 8:41 & 8:49 and agency protocols.
• Serve as a bridge to support patients with SUD and their families in accessing care.
• Support follow-up with a peer recovery coach for patients recently reversed with Naloxone.
• Assist in chronic disease management for conditions like CHF, COPD, diabetes, and hypertension.
• Carry out post-discharge home visits to reduce morbidity and readmissions.
• Educate first responders about resources for SUD patients to improve patient outcomes.
