Job summary

Monogram Health is seeking a Market Physician Executive (MPE) to lead our in-home multispecialty care model across the Charleston market, guiding clinical strategy and operations.

You will oversee frontline clinical and operations teams, review care plans, collaborate with physicians and facilities, and drive evidence-based pathways to improve patient outcomes and reduce costs.

Remote work within the market with occasional in-home visits and travel is expected.

Qualifications

• MD or DO degree from an accredited medical school.
• Active, unrestricted medical license in all applicable states.
• Experience with Medicare Advantage and Medicaid populations.
• Ability to obtain hospital privileges.
• Strong leadership and communication skills.

Responsibilities

• Execute clinical guidelines and approve care plans.
• Lead market operations and oversee daily clinical activities.
• Collaborate with community physicians and facilities.
• Drive Population Health Management to reduce admissions.
• Provide direct patient care when needed.
