Job summary

Monogram Health is seeking a Market Physician Executive (MPE) to lead in-home, multi-specialty care across a New York market. You will partner with APPs, RNs, social workers, and pharmacists to implement evidence-based pathways and manage high-need populations.

This remote leadership role requires MD/DO, board certification, active licenses, and strong governance, with occasional in-home visits and travel to support care delivery and cost outcomes.

Qualifications

• Current state medical license without restrictions; willing to license in multiple states.
• MD or DO degree from an accredited medical school; BC/BE in nephrology, internal medicine, family practice, emergency medicine, critical care, cardiology, endocrinology, hepatology, or geriatrics.
• Active, unrestricted license in each market state; ability to obtain additional states as needed.
• Role is remote within the designated market with occasional in-home patient visits and travel.
• Demonstrated experience applying evidence-based clinical criteria.
• Experience in multispecialty, geriatrics and/or value-based care.
• Advanced management and communication skills.
• Experience with high-need Medicare Advantage and managed Medicaid populations.
• Experience with NCQA, HEDIS, Medicaid/Medicare quality improvement, and risk adjustment.

Responsibilities

• Know, understand, and deliver on Monogram Health’s proprietary evidence-based clinical pathways.
• Ensure adherence to established clinical guidelines and Monogram Model of Care.
• Review and approve APP, RN, SW and Pharmacy plans of care.
• Appropriate and timely patient documentation within Salesforce and Athena clinical activities.
• Review, approve and co-sign APP encounters.
• Overall accountability for reducing total cost of care and Medical Loss Ratio.
• Provide direct and indirect patient care and oversee care delivery.
• Actively lead daily high-risk and concurrent review rounds.
• Direct supervision of front line clinical and operations team members, including shadowing.
• Oversee market operations: patient engagement, scheduling, and P&L management.
• Collaborate with Market Leadership to foster clinical excellence and patient outcome ownership.
• Present market performance to executives; participate in on-call rotation.
• Provide coverage for other MPEs during PTO or vacancy.
