Job summary

Trinity Health Of New England At Home - Connecticut is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes within the Hartford area. You will follow the plan of care prescribed by the physician and overseen by a registered nurse.

Typical schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with weekend/holiday rotation as required. The role emphasizes compassionate, patient-centered care within a mission-driven Catholic health system.

Qualifications

• Graduate from an accredited or state approved school of practical nursing.
• Current licensure in the state of practice.
• Minimum of one year experience in an acute care setting.
• Home healthcare experience preferred.
• Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
• Must have current Driver’s license and reliable transportation to and from work site.
• Ability to consistently demonstrate the Organizational Core Competencies and Organizational values.

Responsibilities

• Provides skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician, and assigned by the supervising registered nurse.
• Monday-Friday, 8a-4:30p, weekend/holiday rotation as required.
