Job summary

Hartford HealthCare at Home is seeking a Revisit Licensed Practical Nurse (LPN) to provide skilled nursing in home and community settings in Torrington, CT. The role operates Monday through Friday, 8:00 AM to 4:30 PM, covering Torrington and surrounding towns with one weekend per month and one major and two minor holidays per year.

Under RN direction, you will follow a plan of care, document EMR progress, and collaborate with various disciplines to ensure high-quality, cost-effective patient

Qualifications

• High School diploma or GED.
• Successful completion of a CT state approved LPN certificate program.
• Current and valid LPN license.
• Valid CT driver’s license & a good driving history.

Responsibilities

• Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
• Record daily progress in EMR.
• Communicate with Primary Nurse and other disciplines.
• Consult with physician regarding client needs and plan of care.
• Assist in evaluation of care given by paraprofessional staff.
