Job summary

Trinity Health Of New England At Home is seeking a Licensed Practical Nurse to provide skilled nursing care for patients in the Hartford area, following the physician's plan and under RN supervision.

This full-time role runs Monday–Friday, 8:00 a.m. to 4:30 p.m., with weekend/holiday rotation as required. Home health experience is preferred; an CT LPN license and reliable transportation are required. We offer comprehensive benefits in a mission-driven setting.

Qualifications

• Current LPN license in the state of practice.
• Minimum of one year experience in an acute care setting.
• Home healthcare experience preferred.
• Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all caregivers.

Responsibilities

• Provides skilled nursing care to clients in home care according to the plan of care prescribed by the physician and assigned by the supervising RN.

About the Position

Provides skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician, and assigned by the supervising registered nurse.

Minimum Qualifications

• Graduate from an accredited or state approved school of practical nursing


• Current licensure in the state of practice


• Must have a minimum of one (1) year experience in an acute care setting.


• Home healthcare experience preferred.


• Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.


• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.


• Must have current Driver’s license and reliable transportation to and from work site.
