Job summary

Hartford HealthCare is seeking a licensed practical nurse for Home Care to deliver skilled nursing services in patients' homes under the plan of care established by the RN Case Manager. You will assess patients, follow through on care plans, and document all visits in a timely manner while collaborating with the care team.

Ideal candidates hold an NLN-approved LPN program, have at least one year of recent medical/surgical nursing experience, and possess CT licensure with reliable transportation

Qualifications

• Graduate of NLN approved school of licensed practical nursing.
• One year of recent medical/surgical nursing experience.
• Current license to practice as a licensed practical nurse in the State of Connecticut.
• Proof of transportation and automobile insurance as required by agency policy.

Responsibilities

• Provides intermittent skilled nursing service to patients and families in accordance with the plan of care.
• Assess the physical and psychosocial status of the patient.
• Follow-through of an appropriate care plan with the RN Case Manager.
• Document all services provided in the clinical record within required timeframes.
• Communicate with RN Case Manager and other team members as needed.
