Isoke Developmental Services LLC seeks Licensed Practical Nurses (LPNs) experienced in behavioral health to support individuals with IDD in in-home and community settings. The role focuses on 1:1 care with duties including G-Tube care, medication administration, and crisis management in a trauma-informed approach.

Ideal candidates have 2–5 years in IDD/behavioral health, CPR/BLS, and a PA or multi-state license.

Formación

• Active LPN license in Pennsylvania or multi-state (MSL) – Required.
• 2–5 years of experience in IDD care, behavioral health, or enhanced support services.
• NADD or NADSP Certification (or willingness to obtain).
• CPR/BLS Certification – Required.
• G-Tube and medication administration experience – Required.

Responsabilidades

• Deliver skilled nursing services in a 1:1 home and community setting per ISP.
• Provide G-Tube care and administer medications.
• Support hygiene and safety needs.
• Implement behavioral support strategies and manage crises using trauma-informed approaches.
• Monitor vital signs and health changes; document through EVV.
• Maintain a safe, structured environment and promote independence.
