Job summary

PruittHealth in Georgia is seeking an LPN Assessment Nurse SOURCE to perform initial and reassessment visits for elderly and disabled waiver program clients in their homes.

You will review medications, document in the Harmony system, and collaborate with the Medical Director and Home First team to ensure services support community living.

Candidates should have a GA LPN license and at least two years of nursing experience with older adults; bilingual translators support is available.

Qualifications

• Nursing degree or LPN license from an accredited program.
• Minimum of two years practical nursing experience with elderly/disabled populations.
• Current and unrestricted LPN license in Georgia.
• Experience with waiver programs preferred.

Responsibilities

• Conduct initial and annual assessments in clients' homes to determine ADLs/IADLs.
• Perform reassessments for transfers, hospital discharges, and added services.
• Review medications for appropriateness and note changes.
• Document assessments in Harmony system per agency guidelines.
• Participate in weekly Medical Director meetings to review cases.
• Maintain safety, privacy, and compliance with regulations.
