Job summary

Pruitthealth Corporation is seeking an LPN Assessment Nurse to conduct initial and reassessment visits for elderly and disabled waiver program clients. The role includes reviewing medications, documenting in Harmony, and coordinating with the Medical Director and Home First team.

The ideal candidate has at least two years of practical nursing experience, holds an active LPN license, and can communicate effectively with translators for non-English-speaking clients.

Qualifications

• Two years practical nursing experience required.
• Current and unrestricted LPN licensure in state of practice.
• Experience working with elders and disabled populations.
• Ability to communicate with translators for non-English-speaking clients.

Responsibilities

• Completes initial and annual assessments in client/member’s home and determines ability to perform IADLs/ADLs.
• Completes modified assessments for transfers, hospital and nursing home discharges, and added services.
• Ensures members receive services to stay in the community per guidelines.
• Communicates monthly with administrator for reassessment schedule.
• Reviews medications for appropriateness and notes changes.
• Documents assessment entries in Harmony per guidelines and requests records as needed.
• Participates in weekly team meetings with Medical Director to review admits, reassessments, hospitalizations, and discharges.
