Job summary

HopeWell Inc. seeks an In-Home Support Worker to assist adults with intellectual and developmental disabilities in community-based living options in Roxbury, MA. This full-time role focuses on promoting independence, dignity, and community integration.

You will assess needs, support goal achievement, and coordinate with families and providers, while documenting services and ensuring safety and compliance. A compassionate, collaborative mindset is essential.

Qualifications

• BA in human services or HS diploma/associate degree with 2 years relevant experience.
• MA in human services is a plus.

Responsibilities

• Collaborate with supervisor, Family Support Specialist, and individuals to set goals in daily living, career needs, social and recreational activities, and parenting when relevant.
• Develop and implement the Individuals' Service Plan (ISP) reflecting preferences, goals, and implementation strategies.
• Build positive relationships with individuals and families to provide direct support and coaching to increase independence.
• Arrange schedules and transport, attend appointments and community activities for individuals and, if needed, their children.
• Provide teaching on personal finance, health, and community resources to promote independence.
• Maintain accurate written documentation and billing records.
• Adhere to safety standards and regulations; collaborate with DDS coordinators and providers.
• Participate in internal and external meetings; communicate with stakeholders.
