Job summary

Youth Haven Services in Person County, NC is seeking a Licensed Professional to join our Intensive In-Home (IIH) program as an IIH Team Lead, serving children, adolescents, and families in home and community settings. The role combines direct therapy with team leadership, supervision, and coordination, including assessments, treatment planning, and ensuring compliance with state guidelines.

Qualifications

• Minimum of one year of documented clinical experience with the population served.
• Valid driver’s license and reliable transportation.
• Ability to travel throughout Person County for home- and community-based services.
• Strong communication skills and professional, ethical behavior.

Responsibilities

• Provide individual and family therapy according to the consumer's Person-Centered Plan.
• Complete clinical assessments and reassessments.
• Offer clinical guidance and direction to IIH team members.
• Conduct weekly team meetings and monitor case progress, interventions, and overall team performance.
• Support compliance for intakes, authorizations, reauthorizations, documentation, and productivity.
• Track treatment outcomes, ensure timely documentation, and assist with transition and discharge planning.
• Collaborate with families, schools, providers, and community partners.
• Perform additional duties as assigned.
