Job summary

Youth Haven Services in North Carolina is seeking a licensed professional to join its Intensive In-Home (IIH) program as a Team Lead serving children, adolescents, and families in Person County.

The IIH Team Lead combines direct clinical care with leadership, team development, and community-based work, with an enhanced rate of $140 per billable contact for services provided in clients' homes and throughout the community.

Qualifications

• Master's degree in SW, Counseling, Psychology, MFT, or related field.
• Current North Carolina licensure (LCSW, LCSWA, LCMHC, LCMHCA, LMFT, LMFTA, etc.).
• Minimum of one year of clinical experience with the population served.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout Person County for home- and community-based services.
• Strong communication skills and professional/ethical behavior.

Responsibilities

• Provide individual and family therapy according to the consumer's Person-Centered Plan.
• Complete clinical assessments and reassessments.
• Provide clinical guidance, direction, and support to IIH team members.
• Conduct weekly team meetings and monitor case progress, interventions, and overall team performance.
• Support compliance with intakes, authorizations, reauthorizations, documentation, and productivity requirements.
• Track treatment outcomes and ensure timely and accurate clinical documentation.
• Assist with transition and discharge planning.
• Collaborate with families, schools, providers, and community partners.
• Support team members in addressing clinical challenges and developing effective interventions.
• Perform additional duties as assigned.
