Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Ravenna, OH. This role offers a flexible schedule and the freedom to achieve professional goals on your terms, meeting patients in their homes with supportive scheduling and training.

Qualified NPs will hold an active State NP license, National board certification, and current BLS.

Qualifications

• Master’s Degree in Nursing or equivalent clinical experience.
• Active State NP license and National board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Provide basic wellness and preventative education.
