Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Logan, OH. This role offers flexible scheduling and the freedom to balance professional goals with personal commitments.

Qualified NPs will travel to patients' homes, perform non-invasive assessments, review histories, document accurately, and provide basic wellness education.

Qualifications

• Master's degree in nursing or an eligible NP credential.
• Active state NP license and national board certification required.
• BLS certification is required or current.

Responsibilities

• Travel to and from patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately and timely.
• Conduct medical reconciliation to ensure safety and compliance.
• Provide basic wellness education to patients.

Education
