Job summary

Hueman is seeking Nurse Practitioners to perform in-home Health Risk Assessments for Medicare/Medicaid members in and around Piqua, OH. This role offers a flexible schedule and a chance to impact patient care beyond the clinic setting.

Qualified NPs will hold an active state license, national board certification, and current BLS, with 12 months of paid advanced clinical experience preferred. Travel to patient homes is required.

Qualifications

• Masters degree or commensurate experience and NP license.
• Active State NP license.
• Active national board certification.
• Current BLS certification.
• 12 months of paid clinical provider experience preferred.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure documentation.
• Conduct medical reconciliation for patient safety.
• Provide basic patient education on wellness and prevention.
