Qualifications

• Active State NP/PA license is required.
• Active National board certification is necessary.
• Preference for 12 months of experience as a paid advanced clinical provider.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness.
