Key responsibilities include traveling to patients' homes, performing thorough non-invasive assessments, and educating patients about preventative healthcare. Candidates are required to hold an active NP/PA license and have current BLS certification.

Qualifications

• Master's Degree or equivalent experience with NP/PA license.
• Active State NP/PA license is mandatory.
• Active National board certification required.
• Current BLS certification is necessary.
• Preferably 12 months experience as advanced clinical provider.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive non-invasive physical assessments.
• Review medical history and ensure documentation accuracy.
• Conduct medical reconciliation for patient safety.
• Educate patients on wellness and preventative healthcare.
