Job summary

Hueman is hiring Nurse Practitioners and Physician Assistants to conduct in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Fort Kent, ME. This role offers a flexible schedule, travel to patient homes, and the chance to make a meaningful impact outside a traditional clinic setting.

Qualified applicants hold an active NP/PA license, board certification, and BLS. Experience is preferred but not required.

Qualifications

• Masters Degree OR commensurate experience and satisfactory NP/PA license.
• Active State NP/PA license and National board certification.
• Current BLS certification; 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare.
