Qualifications

• Master’s Degree or equivalent and satisfactory completion of NP/PA license.
• Active State NP/PA license required.
• National board certification must be current.
• Current BLS certification is necessary.
• 12 months experience as a paid advanced clinical provider preferred.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation for patient safety.
• Provide basic education on preventive healthcare.
