Job summary

Qualifications

• Licensed as a Registered Nurse in New Jersey.
• BSN preferred or must be obtained within three (3) years of hire date.
• Prior experience with patient education, discharge planning, care coordination, or virtual nursing preferred.
• The virtual registered nurse must be a self-directed, proactive professional who can independently navigate barriers, identify solutions, and adapt to evolving workflows while maintaining focus on patient care and operational excellence.
• BLS certification required, as approved by the American Heart Association

Responsibilities

• Assess the health status of assigned patients as outlined in the Nursing Standards.
• Conduct virtual admission/discharge assessments within established timeframes.
• Conduct comprehensive virtual admission assessments by obtaining and reconciling admission information, collaborating with bedside nursing staff to validate assessment findings, identify safety concerns and communicate pertinent clinical information.
• Conduct comprehensive virtual discharge readiness assessments by evaluating patient preparedness for discharge, reviewing discharge medications and plans, confirming follow up appointments, and reinforcing discharge instructions.
• Provide clinical support to inpatient units through remote technology, partnering with bedside nurses to enhance workflows, deliver patient education, monitor changes in patient condition, and escalate issues as needed.
