VCU Health at Home program seeks a Home Health Medic to perform comprehensive assessments, diagnostics, and treatments in patients' homes as part of the Home Hospital model. The role collaborates within an interprofessional team to optimize outcomes and prevent unnecessary hospital use.

Under Nurse Manager supervision, you will conduct daily in-person visits, provide virtual support, and coordinate with nursing and provider teams.

Qualifications

• Minimum of three (3) years of clinical experience.
• Current EMT-P/Paramedic certification/licensure in Virginia; BLS/ACLS.
• Virginia Driver's License in good standing at hire and thereafter.

Responsabilités

• Perform comprehensive assessments, diagnostics, and medical treatments in patients' homes.
• Coordinate care with Home Hospital nursing and provider teams.
• Provide triage and virtual support including calls and telehealth.
• Maintain accurate documentation reflecting assessments and interventions.
• Drive safely and adapt to varying weather and travel requirements.
