The role emphasizes collaboration with the health care team, patient education, and discharge teaching, with strong communication skills and adherence to policy.

Requirements include a Nursing degree (BSN or ADN), VA license or compact state license, and BLS.

Qualifications

• Bachelor's or Associate degree in Nursing required.
• Two years of nursing/patient care in an acute care setting required; five years preferred.
• RN licensure in Virginia or Compact State required; BLS certification required.

Responsibilities

• Conduct patient admission assessments via remote technology.
• Provide patient education and discharge teaching remotely.
• Collaborate with the healthcare team to deliver goal-directed care.
• Document care plans and progress accurately in the EHR.
