Qualifications

• Registered nurse with 2+ years of nursing experience, preferably in home health or geriatric care.
• RN degree from an accredited institution.
• Proficient with Electronic Medical Records (EMR).
• Maintain current BLS/CPR certification and a valid driver's license.

Responsibilities

• Assess and evaluate patient status and write/initiate plan of care.
• Re-evaluate patient and family needs regularly and revise the plan of care as needed.
• Develop care plans with goals based on nursing diagnosis and involve patient/family.
• Administer medications and treatments as prescribed.
• Maintain accurate clinical notes and document changes in patient condition.
• Communicate with physician regarding patient needs and changes in condition.
