Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to deliver in-home hospice care under the Haywood Hospice banner in North Carolina. You will evaluate the plan of care, collaborate with the IDG, and support patients and families facing end-of-life care.

The role requires NC RN licensure, valid driver’s license, and CPR certification; travel within Haywood and Jackson counties is required. Salary ranges between $73,000 and $82,000 per year with a $5,000 sign-on bonus for external

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle.
• CPR certification or ability to obtain within 90 days.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response.
• Collaborate with patient/family, attending physician and IDG for care.
• Instruct patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
• Function in all home settings regardless of demographics.
