Suncrest Hospice is seeking a Registered Nurse (RN) for PRN (as needed) assignments across our service area in Ohio. You will join a dedicated interdisciplinary team delivering compassionate hospice care to patients in homes and facilities through scheduled visits and on-call support.

Ideal candidates hold an Ohio RN license, have strong teamwork and communication skills, and are comfortable using electronic medical records while coordinating with physicians, social workers, chaplains, and other

Formación

• Current Registered Nurse (RN) licensure in good standing in the state of Ohio
• Works well within a team environment with nurses, social workers, chaplains, volunteers and aide leadership
• Can work 1-1 with patients but also has strong support from a patient care team
• Able to communicate by phone, email and written messages, and attend meetings with healthcare professionals
• Time management to schedule visits, documentation and meetings
• Able to learn an electronic medical records system
• Willing to drive with reliable transportation, valid driver’s license and auto insurance to travel across the service area

Responsabilidades

• Provide care and services for a group of patients and visit patients where they reside (homes, SNFs, assisted living)
• Contribute as part of the interdisciplinary team with psychosocial assessments and community resources knowledge
