Job summary

Visiting Nurse and Hospice for VT & NH is seeking a registered nurse to coordinate and manage a home-based hospice caseload. You will assess, plan, implement, and evaluate patient care with the IDT, supervise Home Health Aides, and ensure delivery of comprehensive comfort-focused services.

Responsibilities include nursing visits, documentation, and collaboration with physicians, family, and community resources to maintain quality of care.

Qualifications

• Graduate of an NLN accredited school of nursing.
• Baccalaureate degree preferred.
• RN license in VT required; NH license preferred.
• Hospice/palliative care nursing desirable.
• Minimum two years general nursing experience.
• Hospice/Homecare experience preferred.

Responsibilities

• Provides a full range of professional nursing care to patients in their homes under physician supervision.
• Makes nursing evaluation visits in patient's home.
• Regularly reevaluates patient's nursing needs and informs physician of changes.
• Initiates the plan of treatment and revisions as needed.
• Supervises and schedules Home Health Aides; teaches in-home care.
• Maintains accurate records of patients and care given.
