A local healthcare provider in Bradford, VT is seeking a compassionate Registered Nurse to coordinate and manage hospice patient care. The candidate must have graduated from an NLN accredited nursing school, hold an RN license in Vermont, and have strong communication skills. Responsibilities include providing professional nursing care at home and maintaining accurate patient records. The position requires participation in night, weekend, and holiday shifts as part of a rotating schedule.

Qualifications

• Minimum two years’ general experience in a supervised nursing setting recommended.
• Hospice/Homecare experience preferred.

Responsabilités

• Coordinates/manages a case load of Hospice patients.
• Provides a full range of professional nursing care to patients in their homes.
• Maintains continuity of patient care by planning with IDT members.

Formation

Graduate of an NLN accredited school of nursing
