Job summary

Qualifications

• Minimum two years’ general experience in a supervised nursing setting recommended.
• Hospice/Homecare experience preferred.

Responsibilities

• Coordinates/manages a case load of Hospice patients.
• Provides a full range of professional nursing care to patients in their homes.
• Maintains continuity of patient care by planning with IDT members.
