Job summary

All Care VNA, Hospice & Private Home Care Services seeks a Hospice Registered Nurse (RN) to provide skilled nursing care in patients' homes in Boston and surrounding communities. You will manage caseloads, educate patients and families, and work with an interdisciplinary team to ensure comfort and quality of life.

Responsibilities include assessing patients, planning and updating care, managing pain and symptoms, and thorough documentation in the EMR.

Qualifications

• Active Massachusetts RN license
• 1–2 years nursing experience; hospice/home health preferred
• Strong clinical assessment and symptom management skills
• Excellent communication and interpersonal skills
• Ability to manage a caseload and work independently

Responsibilities

• Perform initial and ongoing hospice home assessments.
• Develop and update care plans with the interdisciplinary team.
• Manage pain and symptoms; administer medications as ordered.
• Educate patients and caregivers on disease progression and end-of-life care.
• Coordinate care with physicians, social workers, and other team members.
• Serve as primary case manager for assigned patients.
• Document accurately in EMR and ensure regulatory compliance
