Job summary

Our Hospice is hiring a Registered Nurse Case Manager (RNCM) for the Columbus team serving Bartholomew and surrounding counties. You will work with a caseload of patients, supported by Nurse Practitioners, Physicians, Aides, Chaplains and Social Workers, visiting patients in the community.

The role operates Monday–Friday with an on-call rotation, 8:00 am–4:30 pm with some flexibility. You will develop and implement care plans, coordinate with IDG, and maintain timely documentation while

Qualifications

• Associate (required) or Bachelor Degree (preferred) in Nursing (ASN/BSN)
• Current Registered Nurse License in Indiana
• Ability to travel via your own vehicle

Responsibilities

• Make routine Hospice nursing visits to patients in home, facilities, hospitals.
• Perform as case manager for assigned patients, manage visits, supplies and reimbursement.
• Teach and counsel patient/family on care techniques, pain and symptom control.
