Job summary

Cape Fear Valley Hospice, part of LHC Group and a member of the Optum family, seeks an RN Case Manager to deliver the Plan of Care for hospice patients with in-home visits. This full-time role requires NC RN licensure, a valid driver's license, and travel within a 50-mile radius.

Working hours are Mon-Fri 8:00-4:30 with on-call rotation. Salary ranges from $75,620 to $113,430 annually, with comprehensive benefits and a supportive, diverse workplace.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and reassesses as needed, at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions and responses to hospice care.
• Collaborates with patient/family, attending physician and IDG members daily.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes.
• Ability to function in any home situation regardless of client characteristics.
