Job summary

Hartford HealthCare at Home is seeking a Hospice Registered Nurse to provide skilled nursing care to clients in their homes or community settings in Connecticut. The role emphasizes symptom management, end-of-life care, and family education within a patient-centered care model.

The ideal candidate will have an Associate’s or diploma in nursing, at least one year of nursing experience, and CT licensure. BSN preferred; home care or medical/surgical experience is a plus.

Qualifications

• Education: Graduate of an Associate’s of diploma program in nursing.
• Experience: One year nursing experience OR one year nursing experience in a specialty.
• Licensure: License to practice in the State of Connecticut.
• Preferred: Graduate of NLN approved BSN program.
• Preferred: Medical/surgery or home care experience.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Conducts a complete nursing assessment based on referral data and clinical observation.
• Develops an individualized plan of care with the client and disciplines.
• Utilizes Hospice Levels of care appropriately and understands payer information for care planning.
• Directs LPNs and Home Care Aides in carrying out the plan of care.
• Teaches patients and caregivers about signs, symptoms and bereavement needs.
• Coordinates all client care services and collaborates with physicians.
