Job summary

Haywood Hospice, part of LHC Group and a proud member of UnitedHealth Group, is seeking a Part-time Registered Nurse Case Manager to provide in-home hospice care.

You will deliver and evaluate the Plan of Care for hospice patients, collaborate with a multidisciplinary team, and support families facing end-of-life care in North Carolina. Flexible 20-hour schedule with occasional on-call coverage and travel within a 50-mile radius.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver’s license, valid vehicle insurance, and access to reliable transportation.

Responsibilities

• Make initial nursing evaluation within 48 hours of referral.
• Identify patient/family needs and develop the plan of care.
• Coordinate and document care with the IDG and other professionals.
• Instruct and supervise patient/family in self-care when appropriate.
• Maintain accurate clinical notes and adapt care as needed.
