Redcrest Hospice in Indianapolis, IN is seeking a Registered Nurse to provide skilled home-based hospice care under medical direction. This on-call role supports patients in their home environment and coordinates with a multidisciplinary team.

Responsibilities include patient assessments, medication administration, chart reviews, patient education, and ensuring regulatory compliance. A current RN license, hospice experience, a valid driver's license, and a designated home workspace are required.

Qualifications

• Current RN license in the state of employment.
• Minimum 2 years hospice experience.
• Ability to work from home with a designated workspace and have a valid driver’s license.

Responsibilities

• Provide skilled nursing care in patient’s home as per plan of care.
• Manage resource applications and chart reviews to support patient needs.
• Educate patients and families and ensure regulatory compliance.
• Complete clinical documentation following agency protocols and Medicare guidelines.
