Job summary

Redcrest Hospice in Indianapolis, IN is seeking a skilled Registered Nurse to provide in-home hospice care. You will assess patient needs, develop care plans, administer medications, and coordinate with physicians and families.

The role involves patient visits, chart reviews, and potential home-based chart support. Eligible candidates bring strong clinical skills, compassion, and meticulous documentation practices.

Qualifications

• Registered Nurse with current license in the state of employment.
• 2 years hospice experience preferred.
• Previous experience with clinical management, hospice case management, or other relevant clinical experience.

Responsibilities

• Provide skilled nursing care in patients' homes as directed by the medical director/attending physician.
• Manage and facilitate resource applications to meet patient needs.
• Support chart review and patient phone calls from home when after-hours needs are minimal.
• Observe and monitor patient conditions and develop an individualized care plan.
