Job summary

St. Gabriel's Hospice and Palliative Care in Texas is seeking a Licensed Vocational/Practical Nurse to provide nursing care to patients at home, coordinate with the Interdisciplinary Team, and support families.

Under RN supervision, you will assess needs, implement care per the plan, document actions, and participate in QAPI and ongoing training. The role requires travel to patient homes, a valid driver's license, and the ability to lift up to 50 lbs; two years LVN/LPN experience and one year

Qualifications

• Two years experience as a Licensed Vocational/Practical Nurse in a clinical care setting and one year full time hospice.
• Graduate of an accredited Diploma or Associate School of Nursing.
• Current State license as a Licensed Vocational/Practical Nurse; Current driver’s license.
• Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Good interpersonal skills.

Responsibilities

• Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation clinical record, team report, and evaluations.
• Participates in planning and implementing care in conjunction with the RN, delivers and documents care based on the Interdisciplinary plan of care
• Provides nursing care utilizing infection control measures (OSHA)
• Assures the continuity of care through delivery of quality patient care.
• Work with the RN and the Interdisciplinary Group/Team to ensure patient/family needs are met.
• Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
• Timely submission of all required paperwork.
• Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and maintaining or improving competency. Participates in the Hospice’s QAPI program.
• Promotes the Hospice’s philosophy and administrative policies.
• Meets productivity standards.
• Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
• Provides effective communication to patients, their family members, team members, and other health care professionals.
