Job summary

Angels Care Hospice in Texas seeks a dedicated Licensed Practical Nurse (LPN/LVN) to provide direct patient care under the supervision of an RN. You will follow the plan of care, administer treatments, document services, and collaborate with the interdisciplinary team to ensure compassionate end-of-life care.

Qualified applicants are licensed in Texas, with 3 years of nursing experience preferred, and a valid driver's license.

Qualifications

• Licensed to practice as LPN/LVN in Texas.
• Minimum of 3 years nursing experience; hospice or med/surg preferred.
• Valid driver's license and reliable vehicle required.

Responsibilities

• Provide direct patient care per the plan of care under RN supervision.
• Administer treatments and medications as prescribed.
• Document patient services accurately and timely.
• Educate patients and families about diagnosis and self-care plans.
• Coordinate with interdisciplinary team to meet patient goals.
