Job summary

Elara Caring is hiring a Licensed Vocational/Practical Nurse to join our home health team in Texas. You will follow patient care plans, document care, and coordinate with physicians and families to ensure safe, effective, and compassionate care in patients' homes.

The role requires LVN licensure, at least 1 year of clinical care experience, and the ability to lift up to 50 pounds. Hospice/home health experience is preferred, with driving to patient locations a regular part of work.

Qualifications

• Current State License as a Licensed Vocational/Practical Nurse (LVN)
• Graduate of an accredited Certificate/Diploma/Associate nursing program
• Minimum 1 year of clinical care experience
• Ability to frequently lift, push, pull, and move up to 50 lbs
• Valid driver's license or reliable transportation to travel between patient homes
• Hospice or home health experience preferred

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Educate patients and families on self-care techniques to support health.
• Collaborate with RN Case Manager to update care plans as needs change.
• Document medication regimens and maintain medication profiles per agency guidelines.
• Communicate with physicians, patients, families, and care team to ensure coordinated care.
• Help prevent avoidable hospitalizations through proactive patient care.
• Deliver compassionate, high-quality care to support positive patient outcomes.
