Job summary

SA Nurses in Texas is hiring Licensed Vocational Nurses (LVNs) to provide in-home care to patients of all ages. You will manage a heavy caseload and deliver compassionate, quality care in patients' homes under the supervision of a Registered Nurse.

The ideal candidate holds a TX LVN license, has 1–2 years of in-home care experience, excellent communication and time-management skills, and a friendly, patient-focused demeanor. Travel between patients daily is required.

Responsibilities

• Under RN supervision, provides quality home care in compliance with nursing standards and policies.
• Delivers direct, individualized nursing care based on physician and RN plans of care.
• Ensures safe delivery of home health services to patients and families.
• Implements planned care and documents activities timely and accurately.
• Educates patients and families about diagnosis and plan of care.
• Maintains a safe work environment and follows clinical standards.
• Coordinates home health services with RN and clinical management.
• Assists patients with learning appropriate self-care activities.
• Uses equipment and supplies efficiently and reports activity as required.
