Job summary

Infinity Hospice Care LLC seeks a Licensed Vocational/Practical Nurse to provide compassionate nursing care in patients’ homes within our Texas service area. You will coordinate with the Interdisciplinary Group and report to the Hospice Administrator while traveling to patient residences as needed.

Under RN supervision, you will identify patient needs, implement the care plan, maintain infection control standards, document care, and communicate with physicians.

Qualifications

• Two years of LVN/LPN experience in a clinical care setting.
• One year of full-time hospice experience.
• Graduate of an accredited Diploma or Associate School of Nursing.
• Current State LVN license and valid driver's license.
• Knowledge of state and federal regulations; good interpersonal skills.

Responsibilities

• Under RN direction, assists in identifying patient needs as evidenced by documentation, team report, and evaluations.
• Participates in planning and implementing care with the RN and the Interdisciplinary Team.
• Provides nursing care with infection control measures (OSHA).
• Assures continuity of care through delivery of quality patient care.
• Works with the RN and Interdisciplinary Group/Team to meet patient/family needs.
• Communicates with attending physicians and hospice physicians involved in care.
• Timely submission of all required paperwork.
• Participates in Hospice in-service programs and QAPI activities.
• Promotes Hospice philosophy and policies.
• Meets productivity standards.
• Performs on-call responsibilities and provides on-call services as assigned.
• Provides effective communication to patients and families, and health care professionals.
• Transportation: Reliable transportation and valid auto liability insurance.
