This part-time, field-based role involves providing direct clinical care and emotional support to patients and families. Applicants must hold an active LPN license in Minnesota and possess previous experience in hospice or home health. Qualificações

• Prior experience in hospice, palliative, nursing home, or home health settings is required.
• Comfortable working autonomously in patients’ homes and coordinating care across disciplines.

Responsabilidades

• Provide direct clinical care including symptom and pain management.
• Collaborate with the interdisciplinary care team to develop care plans.
• Offer emotional support and education to patients and families.
• Maintain accurate documentation in compliance with hospice standards.
• Be available 24/7 for on-call needs, including nights and weekends.
