Job summary

A compassionate hospice care agency is seeking a Licensed Practical Nurse (LPN) for hospice care in Minnesota. In this part-time role, you will provide clinical support to patients in their homes, manage symptoms, and collaborate with an interdisciplinary team. Ideal candidates have an active LPN license and experience in hospice or home health settings. Qualifications

Responsibilities

• Provide direct clinical care including symptom and pain management, vital signs, medication monitoring, and wound care.

• Offer emotional support and education to patients and families navigating end-of-life care.
• Maintain accurate and timely documentation in compliance with hospice standards.
• Be available 24/7 for on-call needs, including nights, weekends, and holidays.

Schedule: Part-Time to Start + Weekend, on-call, and holiday stipulations
