Grace Hospice in Minneapolis, MN is seeking a Licensed Practical Nurse (LPN) to join our interdisciplinary care team. You will provide hands-on nursing care focused on comfort, dignity and quality of life, working with the RN Case Manager and hospice team to manage symptoms and support families.

The role emphasizes education, compassionate care, and collaboration, with home-based care as a core element. This position offers a supportive team and opportunities for professional growth.

Qualifications

• Current Minnesota LPN license.
• Graduate of an accredited practical nursing program.
• Previous nursing experience preferred; hospice or home health experience is a plus.
• Strong assessment, communication and critical-thinking skills.
• Compassionate and respectful approach to patient and family care.
• Ability to work independently while collaborating with the interdisciplinary team.
• Comfortable using electronic health record systems.
• Valid Minnesota driver’s license, auto insurance and reliable transportation.

Responsibilities

• Provide direct nursing care based on each patient’s plan of care.
• Assess comfort, safety and changing needs and communicate updates to the RN Case Manager.
• Support the management of acute and chronic end-of-life symptoms and medications.
• Provide wound care, personal care and assistance with feeding as needed.
• Educate and support patients, families, caregivers and facility partners.
• Collaborate with the interdisciplinary team to provide coordinated, responsive care.
• Participate in bi-weekly interdisciplinary group (IDG) meetings.
• Complete required documentation and follow patient safety, infection prevention, confidentiality and patient-rights practices.
