Job summary

A compassionate hospice care agency is seeking a Licensed Practical Nurse (LPN) for hospice care in Minnesota. In this part-time role, you will provide clinical support to patients in their homes, manage symptoms, and collaborate with an interdisciplinary team. Ideal candidates have an active LPN license and experience in hospice or home health settings. This role offers meaningful work and the possibility to transition to full-time as the patient base expands.

Qualifications

• Prior experience in hospice, palliative, nursing home, or home health settings is required.
• Strong clinical judgment paired with empathy, patience, and excellent communication skills.
• Comfortable working autonomously in patients’ homes and coordinating care across disciplines.

Responsibilities

• Provide direct clinical care including symptom and pain management, vital signs, medication monitoring, and wound care.
• Collaborate with the interdisciplinary care team to develop and implement personalized care plans.
• Offer emotional support and education to patients and families navigating end-of-life care.
• Maintain accurate and timely documentation in compliance with hospice standards.
• Be available 24/7 for on-call needs, including nights, weekends, and holidays.
