Excelin Home Health seeks a dedicated Home Health Registered Nurse (RN) to join our clinical team in New Braunfels, TX. The RN will plan, organize, and direct in-home nursing services, focusing on community health education and patient-centered care.

Responsibilities include initial patient assessments, care planning, medication administration, family counseling, and coordination of discharge planning. A valid Texas RN license and driver’s license are required; prior home health/public health

Formación

• RN with current licensure to practice in the state (TX).
• Graduate of an NLN-accredited school of nursing.
• Must be a licensed driver with insured vehicle as required.
• Minimum of two years’ experience; public health/home care preferred.

Responsabilidades

• Performs initial patient and family assessment to determine home care needs.
• Re-evaluates patient nursing needs regularly and revises care plans.
• Administers medications and treatments as prescribed by physician.
• Counsels patient and family on nursing and related needs.
• Identifies discharge planning needs and coordinates prior to discharge.
• Acts as Case Manager for assigned caseload.
