Job summary

Excelin Health Services in Houston, TX seeks a Registered Nurse (RN) for full-time clinical nursing in the home health setting. The role focuses on planning, directing, and delivering patient-centered home care services within the community.

The RN will assess, plan, implement, and evaluate nursing care, educate patients and families, and coordinate with physicians to ensure optimal outcomes. A valid RN license and two years’ public health/home care experience are preferred.

Qualifications

• Self-directed with minimal supervision.
• Strong verbal and written communication skills.
• Ability to organize and prioritize workload independently.
• Experience supervising home health aides not required but helpful.
• Must be able to lift and move patients and equipment.

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Re-evaluates patient nursing needs regularly.
• Initiates and revises the care plan as patient status changes.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family on nursing and related needs.
• Provides care instructions to patients per plan of care.
• Identifies discharge planning needs and coordinates care prior to discharge.
• Acts as Case Manager and coordinates patient care for assigned caseload.
