Qualifications

• Active RN license, unrestricted.
• At least 1 year of recent home health RN experience.
• OASIS documentation experience.
• Proficiency with HomeCare HomeBase preferred.
• Reliable transportation and valid driver's license.
• Ability to independently manage a full-time, structured schedule.

Responsibilities

• Deliver skilled nursing care and health interventions in patients' homes.
• Conduct comprehensive patient assessments and complete OASIS documentation.
• Collaborate with physicians and the care team to develop and implement care plans.
• Provide wound care, manage medications, and monitor chronic conditions.
• Educate patients and families on disease management and care strategies.
• Communicate changes in patient status to the clinical team promptly.
