Job summary

BrightStar Care seeks a compassionate Home Health Registered Nurse (RN) to provide high-quality care to patients in their homes across Amarillo, TX. You will perform Start of Care visits, comprehensive assessments, care planning, wound care, and lab draws, coordinating with physicians and the care team to ensure optimal outcomes.

This role offers autonomy, strong relationships with patients and families, and the opportunity to improve independence and comfort in the home setting.

Qualifications

• RN license in Texas
• CPR/BLS certification
• Minimum one year nursing experience
• Strong assessment, organization, and communication skills
• Valid driver's license and reliable transportation

Responsibilities

• Perform Start of Care, Recertification, Resumption of Care, Evaluation and Discharge visits.
• Conduct comprehensive nursing assessments and develop plans of care.
• Monitor patient progress and communicate changes to the physician and care team.
• Coordinate care with physicians, therapists, patients, caregivers and community resources.
• Assess home health eligibility for therapy services (PT/OT/ST) and coordinate evaluations.
• Perform routine and specialized nursing procedures, wound care, labs, medications, and chronic disease management.
• Educate patients and caregivers on disease processes, medications, wound care, safety and self-management.
• Complete accurate electronic documentation and ensure HIPAA compliance.
